Step 5: Click the purple + next to “All Pages.” Select “Public Page” and give it a clear name.

Step 6: Add a form view and click on the table you created.

Step 7: Using the menu on the left-hand side, edit the form view’s settings, field inputs, actions, and rules. You can also create emails to be sent automatically to users after a submission is complete.
